Author Notes: Colonel John Hickman is Emeritus Reader in Animal Surgery of the University of Cambridge. He was the founder President of the Bristish Equine Veterinary Association and the first Editor if the Eqine Veterinary Journal. He wrote Veterinary Orthopaedics, was co-author of An Atlas of Veterinary Surgery and edited Horse Mangement and Equine Surgery and Medicine.
